Newborn Resuscitation: Meconium Aspiration Syndrome
نویسنده
چکیده
At present time, delivery room management of babies born through Meconium stained amniotic fluid (MSAF) often includes suctioning of the mouth and nose before the delivery of the shoulders. This addresses the concern that babies will inhale meconium present in the upper airway with their first breath putting them at risk of MAS. The rationale of routine perineal suctioning of all babies born through MSAF has recently been questioned. Likewise, tracheal suctioning of meconium in babies born through MSAF is an established intervention in many delivery rooms. However, disagreements exist concerning the value of perineal and routine endotracheal suction of the airway in the delivery room management of vigorous meconium-stained infants. Methods

Meconium aspiration syndrome in neonates
Perhaps the most significant changes in airway management over the recent years have been in the management of an infant delivered through meconium- stained amniotic fluid( MSAF). MSAF occurs in approximately 10℅ to 20℅ of all deliveries and increases to over 30℅ in deliveries after 42 weeks gestation. Meconium aspiration syndrome ( MAS) occurs in about 2℅ to 5℅ of these cases with a high morta...

In 2006, the American Academy of Pediatrics and the American Heart Association published the 2005 guidelines on neonatal resuscitation. Before the 2005 guidelines, management of a newborn with meconium-stained amniotic fluid included suctioning of the oropharynx and nasopharynx on the perineum after the delivery of the head but before the delivery of the shoulders. The 2005 guidelines did not s...
متن کاملRespiratory distress in the newborn.
The most common etiology of neonatal respiratory distress is transient tachypnea of the newborn; this is triggered by excessive lung fluid, and symptoms usually resolve spontaneously. Respiratory distress syndrome can occur in premature infants as a result of surfactant deficiency and underdeveloped lung anatomy. Intervention with oxygenation, ventilation, and surfactant replacement is often ne...

Neonatal pneumopericardium (PPC) is a rare form of neonatal air leak syndrome with high morbidity and mortality. Air leak syndrome in the newborn is usually associated with active resuscitation, respiratory distress syndrome, meconium aspiration syndrome, mechanical ventilation, or trauma associated with labour. Neonatal PPC can be associated with other air leak syndromes such as pneumomediasti...
متن کاملPrevention of meconium aspiration syndrome: an update and the Baylor experience.
The approach to preventing meconium aspiration syndrome (MAS) in the newborn has changed markedly over the last 30 years. In the late 1970s, all infants born through meconium-stained amniotic fluid (MSAf) had upper-airway suctioning before delivery of the shoulders and then had tracheal intubation and suctioning in the delivery room. Now suctioning of the upper airway is no longer recommended, ...
